Health History for Sports Participation
In order to participate in school sports, you must have a current physical (1 year old or less) on file in the Health Office. If your current physical is more than 30 days old at the start of the sport, the parent/guardian must complete an Interval Health History Update form and return it to the Health Office before sports clearance can be obtained.

At the Beginning of the School Year
Just a reminder to parents that physicals are required for the following grades:

  • PreK, K, 2, 4, 7, and 10 and any student new to the District.
  • Sports physicals are required annually.
  • Please be sure to include dental certificates with the physicals.
Also, a reminder that students entering 6th grade are required to have a Pertussis Booster PRIOR to the start of school in September.

Please keep these requirements in mind this summer when you visit your child's pediatrician. If you have questions regarding these requirements, contact the Health Office at 639-5594 ext. 52029
