ATHLETICS INFORMATION
Every student must have a completed SPORTS PACKET each season! Click here for the forms you will need. This has been updated for the 2022-2023 athletic season. These forms must be signed by a parent/guardian. In order to participate in school sports, you must have a current physical (1 year old or less) on file in the Health Office. Please call if you are unsure if a physical is up to date.

PHYSICALS AND OTHER REQUIREMENTS
-Physicals are required for the following grades: PreK, K, 1, 3, 5, 7, 9 and 11 and any student new to the District
-Sports physicals are required annually (see above)
-Please be sure to include dental certificates with the physicals
